We provide an outreach support service to enable older people to live safely and independently in their own homes and improve the quality of their lives.


The support is tailored to each person's needs whilst promoting dignity, choice and independence.


Our outreach team will:


  • Advice on personal safety and security, including providing links to community alarm services.


  • Provide access to aids/adaptions/equipment within the home allowing service users to maintain their independence and wellbeing.


  • Advocate on behalf of service users ensuring that we represent their interests and maintain their rights.


  • Signpost to agencies who can advise on accommodation, home maintenance and repairs.


  • Home visits; accompany people to appointments and putting service users in touch with useful services.


  • Interpret where African Caribbean elders have reverted back to their native language and dialect.


  • Support service users to access alternative accommodation when their current accommodation is no longer suitable.


  • Provide access to information to enable service users to make informed decisions around health matters.


  • Offer practical and emotional support.


  • Provide guidance on how to maximise income by advising on benefits available.


  • Assist with writing letter and filling out forms.


  • Arrange for community care assessments by social services


  • Befriend service users to alleviate feelings of isolation and loneliness.


  • Organise social activities e.g. coffee mornings, day trips, educational trips and outings etc.
